We can help you find things of interest and take part in different activities where you live.
Arranging and attending health appointments
We can help you arrange and attend health appointments. For example, a doctor, dentist or optician. And we can help you with medication.
We can support you to understand and deal with any correspondence you receive including letters and emails.
We can help you write your shopping lists and go shopping with you. Or we can help you arrange independent travel so you can go shopping alone.
We can support you to find out which benefits you may be entitled to and to apply for them.
Building positive relationships
We will support you to build positive relationships with other people. That may be people that you live with, go to college with and your friends and family.
Financial budgets and paying your bills
We can help you work out a financial budget so you know how much money you have coming in and how much you will have to pay out. And we can support you to pay your bills.
Education and work
If you would like to explore educational and employment opportunities we can help you find something suitable, apply for a place and plan when and how you will get there.
We can support you to consider the different housing options available to you and to apply for housing. If you move home we can support you with things like packing, removals and updating your address.
Maintaining your tenancy
If you rent your home and you have a tenancy agreement, we can help you to understand what it means and to live within the terms and conditions of it.
We can help you to keep your home clean and tidy by supporting you with things like washing up, laundry and hovering.
Meal planning and cooking
We can support you plan your meals to ensure a balanced diet and we can help you with cooking.
A person-centered approach ensures people who need support are recognised as individuals with strengths, preferences, rights and choices over how they live their lives.
We use person-centered thinking tools when designing care packages and care plans. Our services users can:
- help to design their care plan
- have a say about who supports them and when
- participate in support reviews every 6 months, or whenever they request one
What you can expect from us
- we work hard to provide care and support to our service users. We are regulated by the Care Quality Commission (CQC), Medway Council and Investors in People
- our staff are professional and trained to deliver services that are person-centred
- we offer Residential Care and Supported Living services in Medway and Kent and we can work with people in their own homes
- we will support our service users to work towards greater independence
